Waist-to-Hip vs. Waist-to-Height: Which Ratio Should You Track?
Two simple tape-measure ratios, two different jobs. Using the right one — and not confusing them — sharpens what your measurements tell you.
A tape measure is one of the most underrated health tools you own. With it, two simple ratios — waist-to-hip ratio (WHR) and waist-to-height ratio (WHtR) — give you more insight into cardiometabolic risk than the bathroom scale ever will. They’re often mentioned in the same breath, but they measure different things and are best used for different purposes. Knowing which is which keeps your numbers meaningful.
Why ratios beat raw waist size
Waist circumference alone is informative, but it doesn’t account for body frame. A 36-inch waist means something very different on someone who is 5'2" than on someone who is 6'4". Ratios solve this by putting your waist in context — relative to your hips or your height — making them more comparable across body sizes.
Waist-to-height ratio (WHtR): the cardiometabolic screen
How to calculate it: waist circumference ÷ height, in the same units.
WHtR is fundamentally a measure of central (abdominal) fat relative to your frame. Because abdominal fat — especially the visceral fat packed around your organs — is the metabolically dangerous kind, WHtR is a strong, simple proxy for cardiometabolic risk.
Its great strength is a memorable rule of thumb: keep your waist to less than half your height. A WHtR under 0.5 is the general target. Risk rises as the ratio climbs:
- Below 0.40: may indicate insufficient reserves; generally low risk in the healthy range.
- 0.40–0.50: healthy range for most adults.
- 0.50–0.60: elevated risk; central fat is accumulating.
- Above 0.60: high risk.

Waist-to-hip ratio (WHR): the fat-distribution metric
How to calculate it: waist circumference ÷ hip circumference, in the same units.
WHR captures something different — the distribution of your body fat, comparing where you store fat around the middle versus around the hips and glutes. It’s the classic “apple versus pear” shape indicator:
- A higher WHR (“apple” shape) means proportionally more fat carried around the abdomen, associated with greater metabolic risk.
- A lower WHR (“pear” shape) means proportionally more fat on the hips and thighs, which is metabolically more benign.
General thresholds for elevated risk are roughly above 0.90 for men and above 0.85 for women, though these vary by population. Because hip circumference partly reflects muscle and bone structure, WHR is less a pure measure of “how much abdominal fat” and more a measure of “what shape your fat distribution is.” This makes it especially useful in body-composition classification — including functional-medicine phenotyping that uses it to characterize body type — rather than as a standalone risk gauge.
The key difference, in one line
- WHtR answers: How much central fat do I carry relative to my frame? → best for screening cardiometabolic risk.
- WHR answers: What is the pattern of my fat distribution? → best for characterizing body shape and composition type.
They’re complementary, not competing. WHtR tells you whether your waistline is in a healthy zone; WHR tells you about the apple-versus-pear pattern behind it.
Which one should you track?
For routine self-monitoring of health risk, track WHtR. It needs only one body measurement plus your height, it has a clear and memorable target, and it correlates strongly with the metabolically harmful fat that drives heart disease and diabetes risk. It’s the single most practical number for most people.
Bring in WHR when you want a fuller picture of your body composition or shape — for example, when classifying body type or when a clinician is assessing fat-distribution risk. Measuring both takes seconds and gives you two complementary readings.
Measuring accurately matters more than which ratio you pick
Any ratio is only as good as the measurements behind it. For consistent results:
- Measure your waist midway between the bottom of your ribs and the top of your hip bones, after a normal exhale, without compressing the skin.
- Measure your hips at the widest point around your buttocks.
- Keep the tape level and snug, and measure under the same conditions each time.

The bottom line
Waist-to-height ratio and waist-to-hip ratio are both quick, powerful, nearly free health metrics — but they do different jobs. Use WHtR as your primary cardiometabolic screen, anchored by the simple goal of keeping your waist under half your height. Use WHR to understand the shape and distribution of your body fat. Measure carefully, track the trend, and these two ratios will tell you more about your health trajectory than your weight ever could.
